Advanced multimodal assessment of pulmonary arteries in chronic thromboembolic pulmonary hypertension treated with PTPA/BPA

On January 31st an international conference focused on advanced multimodal assessment of pulmonary arteries in chronic thromboembolic pulmonary hypertension treated with PTPA/BPA commenced by the Department of Internal Medicine & Cardiology with the Centre for Management of Venous Thromboembolic Disease took place.

The Library and Information Centre of the Medical University of Warsaw served as the venue for the conference which included specialists representing leading world, European and Polish centers which employ advanced modalities of intravascular imaging such as cone beam CT, intravascular ultrasonography, optical coherence tomography, and pressure wires that allow for the plausible visualization of thrombi in the pulmonary arteries and for the proper evaluation of their hemodynamic influence.

During the meeting distinguished speakers, Prof. Hideaki Yoshino and Prof. Toru Satoh, the pioneers of refined balloon pulmonary angioplasty (PTPA) from the Kyorin University School of Medicine in Tokyo presented long term results of their experience with PTPA in the treatment of chronic thromboembolic pulmonary hypertension (CTEPH).

Prof. Bernhard Meyer , the head of the Interventional Radiology Department for the Hannover Medical School, one of the leading centres in treatment of pulmonary hypertension and CTEPH presented advanced imaging methods of the pulmonary arteries based on angiography and tomography.

Prof. Jolanta Kunikowska from the Nuclear Medicine Department and Dorota Piotrowska-Kownacka, MD from the I Radiology Department of the Medical University of Warsaw shed light on the application of lung scinthigraphy and Cardiovascular magnetic resonance in the diagnostic work-up and monitoring of CTEPH patients.

Renowned guests from Japan: Takumi Inami, MD, Haruhisa Ishiguro, MD and Yohei Shigeta, MD, experienced operators who perform PTPA sheared their practical advice on employing pressure wires and OCT.

Specialists form Polish centres for CTEPH treatment: Prof. Grzegorz Kopeć from the Department of Cardiology of the Jagiellonian University Medical College, Prof. Aleksander Araszkiewicz from the Department of Cardiology of the Poznań University of Medical Sciences and Marek Roik, MD from the Department of Internal Medicine & Cardiology with the Centre for Management of Venous Thromboembolic Disease headed by Prof. Pruszczyk described their experience in using advanced imaging modalities in patients treated with BPA.

The Catherization Laboratory of the Department of Internal Medicine & Cardiology with the Centre for Management of Venous Thromboembolic Disease, as the first center in Europe has been performing PTPA monitored with IVUS, OCT and PPR using pressure wire since 2014. These state-of-the-art imaging methods allow for the safety and efficacy of balloon angioplasty of pulmonary arteries, a cutting edge therapeutic option for patients with CTEPH. To this day, our Catherization Laboratory has performed procedures in 25 patients with satisfactory results and minimal complication risk.
